A psychological assessment is completed in the client’s home.

A Social Worker identifies the needs of the client.

Identify what level of care the client will require. The client will need to be semi- independent for consideration of an assisted living facility.

Based on the client’s physical condition, short term or long term care will be determined.

Financial assessment completed to determine client’s ability to pay for services.

Counsel with client regarding their concerns about assisted living, their preference of location, their financial responsibilities, and what unmet needs will exist if they consent to go into an assisted living facility.

Identify geographical desired location for client.

Help the family and client in decision making and long range plans.

If the client requires placement in an assisted living facility a referral to case management is necessary in order to implement the process.

A signed contract and authorization to act as representative is required.
