Case Management
Identification:
The first step is to identify the need for case management
and the appropriate person(s) to be responsible for managing the
case. To identify what the client needs are and if the agency is
capable of meeting those needs. This entails a comprehensive
assessment of clients functional physical, mental and social
status to identify needs, what is the client and family
expectations of the case manager, and most of all what the client
wants the case manager to handle.
Assessing and Planning:
This step assesses the individuals condition to determine
what services and resources are necessary and to plan for their most
appropriate delivery.
Coordination and Referrals:
The ability to identify and coordinate the many diverse
elements of a treatment plan is the core of case management.
The case manager will refer the client to the most appropriate
provider and if necessary, recommends various social, financial
and other services that may be desirable in addition to medical
care.
Action Plan/Treatment:
The implementation of the treatment plan is the fourth step
in case management. Although case management does not participate
in the actual delivery of services, i.e. homemaking services,
cooking the meals, providing personal care, gardening,
transportation and chores they serve as the central focal point
for centralized communication and coordination.
