Over the span of 25 years Alma Fortson has gathered her experience from employment in a Level One trauma hospital, home health agents, insurance companies, Dialysis Centers and nursing homes. The company and its work ethics are based on sound social work ethics, strong technical knowledge, current community base knowledge and a personal and professional desire to provide the best services possible.

Assessment Defined:
An assessment is the process in which a professional degree individual from a School of Social Work investigates with the client their physical, emotional and environmental status. The client is made aware of their present living conditions. This is performed by skilled knowledge and observation of the client's physical, environmental and cognitive status. Physical observation are conducted based on client's past two years life style, to determine if there has been a slow, rapid or steady deterioration of physical functions. A friend or relative may need to be consulted for comparison. Cognitive suitability is assessed based on the Social Worker's knowledge of mental health, diagnosis and the disease process. The client's personal hygiene and their ability to maintain themselves in the home is a critical factor in the assessment. Environmental assessment is completed looking at the physical upkeep of the property, home and the immediate living space. When the assessment is completed the Social Worker focuses on what the client considers their problematic areas to be. Problematic areas are sometimes what clients considers powerless to control.
The Social Worker will identify problems or concerns based on the assessment and the client's perceptions of their needs. The needs are prioritized for expediency. For each problem we will identify a solution. When there are no solutions we will identify that also. The assessment does not allow the Social Worker to investigate services for future use, to implement services or to make changes in the client's lifestyle.
To obtain services you will need to implement a referral to the Case Management.
Case Management services requires a signed contract and a deposit. A detailed report is submitted to the purchaser within ten days.
Why Retain A Case Manager?
For The Client:
Most elderly people have little knowledge of services within their community that they can access. They have limited knowledge of health care providers and procedures and they may find it confusing and sometimes a little intimidating and overwhelming to negotiate services from health care, private agents, state or federal programs. The case manager is the negotiating leverage that gives the client the best chance for achieving their highest level of functioning.
For The Family:
The responsibility for overseeing the client's activities of daily living usually falls to the family or significant other. However, family members or loved ones are often unprepared to negotiate with agents, research services in the community, implement services then monitor those services. Fortson Consult Services can help you with all those day to day activities that must be taken care of.
Case management assures the family that the client is receiving the most appropriate form of treatment. Direct observation of the client helps the family and the client make critical decisions that are based on skilled observations of a license clinical social worker.
