Ability to Live Independent:
The assessment is done in the client's home.
1. Assess the needs based on the client's physical condition and their ability to negotiate their environment.
2. Evaluate client's ability to live alone. This is conducted by a skilled Social Worker, evaluating the client's cognitive status, physical status, and their ability to perform daily activities of living. These activities consist of personal care, shopping needs, obtaining needed services, ability to maintain property / home and transportation needs.
3. Present all options that are available for changes in life style, i.e., assists living, companion living or nursing home placement.
4. Present the cost and eligibility for all the services discussed, companion, assisted living and nursing home.
This assessment does not allow the Social Worker to make any changes in the client's lifestyle or to search for an assisted living or nursing home. The client will need to be referred to the Case Management Program in order to implement any services.
