Insurance Company Assessment.
1. The assessment is conducted in the client's home, any support person that is available is welcome to be present at the conference.
2. A psychosocial evaluation is completed.
3. The social worker identifies needs through conversation as to what the patient perceive their needs to be.
5. Social worker explores options with the patient such as, inadequate in home assistance, need for Respite care, additional help at home, Day care centers, assistance with medications, need for DME equipment, and transportation to meal sites for socialization.
6. Based on patient's needs the social worker explains what resources are available to address those needs. If the patient requires nursing home placement the social worker will determine if there is any skill coverage for the nursing home to accept the patient under their insurance. If the patient has not been in the hospital within thirty days the social worker will need to determine a source of payment. There are three sources of payment, insurance/ medicare, private care and Medicaid. Medicaid is a state funded program. If the patient requires placement in a nursing home under Medicaid several interventions will be required.
7. If the patient requires a resource from the community, the social worker will assess the availability and make the appropriate referral.
8. Financial assessment is completed if the patient is asking for assistance from the county or the state to obtain a needed service.
9. A treatment plan is created for the patient for long term planning.
