Nursing Home Placement Assessment

There are many components of attempting to or the actual placement of an individual into a nursing home. In Florida there are basically four ways to enter a nursing home; Private pay, Medicare, Medicaid and Insurance sponsored. The License Clinical Social Worker explore all options that are appropriate for the client. The doctor may order a client to be placed in a nursing home, but the care required will determine the source of payment.

Assessments are completed in the client's home.

A psychosocial assessment is completed.

The Social Worker identifies if the client is appropriate for a skilled facility.

When you are identifying skill care, you must be able to recognize care that can only be provided by a license registered nurse or a license practical nurse.

Clients unable to feed self, unable to ambulate to bathroom, unable to take their medications, can not be left alone in fear of falls, unable to prepare their meals is not considered skilled care that Medicare and Insurance companies are willing to pay for in a nursing home.

The Social Worker identifies the level of care the client will require for placement.

The social worker will determine at the time of the assessment if short or long term placement is required.

Assess the client's financial status and determine a payment source for placement in the nursing home.

Counsel with client regarding their fears of placement, their preference of locations, their financial responsibilities, and what unmet needs will exist if they are placed in a nursing home.

If the client requires a state funded program for nursing home payment, explain to the client all the steps involved, the application process, financial screening, the waiting list and what is required for eligibility.

If the client requires placement in a nursing home after the assessment is completed and submitted, the purchaser may make a referral to case management.

The evaluation fee is waived and a deposit is required.

A signed contract and authorization to act as representative is required.
