Personal care provided by Fortson Consultant Services is less expensive than the cost of an airline ticket. We’ll step in and provide the same level of care as you would if you were here. So relax and let FSC take care of your loved ones. This service package is designed for an individual who is alone and is being discharged from the hospital. The client will be provided the following services.

1. The case manager will meet with the client in the hospital prior to discharge. The purpose of this visit is to get acquainted with the client and determine what their needs will be upon discharge. Also at this time the case manager will determine if we can accept the case.

2. If the case is accepted, the case manager will meet the client at their home when they are discharged from the hospital. If for what ever reason we are not able to accept the case we will refund your deposit except for $75.00. The $75.00 covers the social workers trip to the hospital, a written needs assessment to the purchaser.

3. Eight hours of companion/homehealth services is provided for client to make the transition from hospital to home.

4. Within the eight hours, the client will be continuously assessed to determine on-going needs. Light meals will be prepared, personal care will be provided, grocery shopping for immediate needs taken care of, medications picked up from the pharmacy, client bedroom prepared for comfort and convalescing.

5. An assessment of client’s physical, environmental, emotional status strength and weaknesses are recorded and submitted to the requester of this evaluation.

6. The eight hours of companion/home health services may be split into four (4) hours each, no other division or split is acceptable. The cost for transportation from the hospital to home, medications, purchasing of food is the client’s financial responsibility.

$75.00 fee for needs assessment if the case is not accepted.
